Certification Application Portal – General Information for CWI Program
IMPORTANT NOTE 1: Candidates applying for a certification exam, retaking an exam, renewal, or recertification:
To streamline the application process, please prepare all of the required application documentation prior to starting the application submission process.
Based on the specific Certification application (Initial, Retaking an Exam or Renewal/Recertification), specific application information will be required as listed below.
IMPORTANT NOTE 2: Please do not proceed with creating an application until you have ALL appropriate documentation digitally available for upload!
IMPORTANT NOTE 3: File size for each document upload cannot exceed 5.12 MB (or 5120 Kilobytes).
Please be aware that the only acceptable payment methods available through the portal are credit card and ACH (Automated Clearing House Payment Method).
Retaking an Exam(s)
Proof of Identity (Required in PDF) − Upload a copy of an official government passport or national ID (e.g., driver’s license).
* Proof of ID only required if scheduling an exam(s).
Visual Acuity Form (Required in PDF)
− Download and complete the Visual Acuity
form. An eye examination must be administered by an Ophthalmologist, Optometrist, Medical Doctor, Registered Nurse, or Certified Physician’s Assistant.
ADA (when applicable)
− Download and complete the ADA Disability Accommodations
form. Candidates will be required to upload this form in PDF format during the application process if requesting accommodations.
Employment Verification (Required in PDF)
− Download and complete the Employment Verification
form. This form must be completed by your most recent employer.
Qualifying Work Experience − Candidates will be required to enter relative work experience which is applicable to the program. Candidates may need to enter more than one employer; therefore, please make sure that you have the following information ready for each employer entered:

6. Supervisor’s Name

Education (when applicable) − Candidates who are substituting work experience may upload a copy of their transcripts or diploma in PDF format during the application process.
− Candidates who require training must submit proof of their hours in PDF format. If training is not required, please download, complete and upload the Training Waiver
to bypass this training requirement.
Retest Opportunity − Candidates are permitted to retake up to three (3) exams within three (3) years of the original exam(s) date. Applicants may retake the first exam(s) of the segment(s) that did not result in a passing score without additional training. Any additional retaking of exams (2nd or 3rd) shall require documented evidence of additional training – 16 hours for a 1-part or 2-part re-exam(s); 40 hours for 3-part exams.
Professional Development Hours (when applicable) − Candidates recertifying by PDHs will be required to upload documentation that verifies your hours in PDF format (example: certificates of completion with course descriptions).
