Test Facility Instructions
The following outline provides the information necessary for a facility to become an AWS Accredited Test Facility:

• Complete and submit the QA Manual and checklists to Alfred Nieves, AWS, Certification Business Unit, 550 NW Le Jeune Road, Miami, FL 33126. Include application fee, two copies of the facility's Quality Assurance Manual, one controlled copy and one uncontrolled copy. Also include the completed QA Manual checklist and the On-Site Audit checklist. The current application fee is $400.00. Please make the check payable to: American Welding Society.
• An invoice for $1,100.00 will be issued to the facility following a satisfactory review and document approval process. The invoice and payment needs to be mailed to: AWS, Certification Business Unit, P. O. BOX 440367, Miami FL 33126. After the invoice is paid AWS will schedule an on-site assessment of your facility by a third party assessor.
• After the assessment audit is completed the facility will receive notification of their results -- including the deficiencies requiring corrective action, an invoice will be issued to the facility for travel expenses incurred by the third party assessor.
• If the results are satisfactory, a certificate of conformance will be issued after AWS receives your check. The certificate allows your facility to operate in accordance with the AWS QC4-89, Standard for Accreditation of Test Facilities for the AWS Certified Welder Program.
